I think (and hope) that he was reffering to the fact that the first three causes are all parts of the same proces (cell tries to adapt: phase 1, cell fails and gets injured: phase 2, cell dies due to extensive injury: phase 3)

These are not causes.. ask your self what causes those? What causes Adaptation, Injury etc. These are symptoms.
